Facial Contouring

Dermal Fillers

Filler injections are used to smooth out wrinkles on the face. The newest and safest filler is Hyaluronic Acid (H.A). It is injected not only to fill lines (e.g., Laugh lines and Marionette lines), but to volumize and to reestablish the youthful contour. The aging process begins due to the loss of padding beneath the skin (e.g., diminishing of baby fat). In addition, elasticity and fullness are also compromised due to diminution in new collagen formation. The loss of padding and volume varies with the individual. A dermal filler, when used correctly, raises up the skin in the area of the injection.

Certain areas of the face evidence the loss of contour most dramatically:

  • Front of the cheek
  • Malar prominence
  • Center of the cheek
  • Temple Area
  • Tear Trough
  • Lower face (below and lateral to the mouth)
  • Upper and lower lips-enhance and augment
  • Jawline

As padding diminishes, the skin in these areas flattens or sinks and the face begins to sag and wrinkle. Genetics, age and body weight are all factors in determining the extent of treatment required. It is very important that the product used is safe and biodegradable, and that its effect if deemed necessary is reversible. With the right filler as a tool and a clear model in mind, I am able to work as an artist to volumize the face and restore its youthful contour. Some rejuvenation is obvious after the first treatment, and so is the gratification it brings.

Dermal Fillers include:

  • Hyaluronic acid (Restylane, Juvederm, Perlane). The hyaluronic acid draws fluid to the treated area.

Dermal Filler Treatments

Dr. Kanodia first numbs the area selected for treatment with a local anesthetic (ice may be used as well). A needle is then used to inject the filler substance beneath the skin. Total time spent for the treatment should be no longer than fifteen minutes. It’s quick and relatively painless.

What is Botox?

Botox, or when used for cosmetic purposes Botox Cosmetic is a type of neuromodulator that relaxes muscles to soften and eliminate certain lines and creases. Relaxing these muscles effectively flattens out that area of skin giving a more youthful and aesthetically pleasing appearance.

Contrary to rumor, Botox properly used does not “freeze the face” and eliminate its expressiveness. Here again, achievement of a natural-looking result is as much art as science. The objective is not simply youthfulness, but a youthfulness in accord with the individual face. Botox injections should be done very gently and carefully so as to prevent damage and deep lines; it can be very precisely filtrated and calibrated to the needs of the client. It is of special importance to actors (although really no less so to the rest of us) that the face remain fully animated and expressive.

How Botox is Injected

Botox can be administered at Dr. Kanodia’s office by appointment only. Before your appointment the injection will be freshly prepared to ensure optimal results. Next, Dr. Kanodia will consult with you and discuss your concerns as well as inspecting the areas to be injected. Once deciding on the proper areas for treatment, a small amount of Botox will be injected directly into the site where results are desired through a tiny needle. It’s relatively painless and takes only minutes.

Primary Areas Targeted by Botox

  • Frown Lines (Procerus and Corrugator)
  • Horizontal Lines in the forehead (Frontalis)
  • Crows Feet (Orbicularis Occuli)
  • Neck (Platysma)- to prevent downward pull

Secondary Areas Targeted by Botox

  • Vertical lines above the upper lip (very carefully and superficially injected with micro drops using a 32 gauge needle). Here I use 1/5th of what is typically used, causing no interference with the mobility of the mouth.
  • Mentalis–in the chin to soften the orange peel appearance.
  • The jowl area (Depressor Anguli Oris)–when the D.A.O. muscle is strong it has a tendency to pull the face down and create the appearance of jowling. Preventative treatment can be started as early as age 30. 3-5 units of botox on each D.A.O. is safe and adequate.
  • Under the eye–1 micro drop (2-3 units) to help lines under the eye in selected patients.

Facelift

The face lift is one surgical option, but Dr. Kanodia only recommends this procedure for those individuals experiencing jowling (bunching of skin along the jaw line), and folding of skin around the neck.  A neck lift, which is usually done in conjunction with the face lift, is an effective means of correcting these problems.  When properly performed, face and neck lifts become a unified operation that produces a more youthfull re-draping of muscle and skin.

In fact, a good face lift should result in the patient looking between seven to ten years younger.  When performing a face lift, Dr. Kanodia takes great care to create a natural look that doesn’t distort or constrain facial expression.  The procedure, which usually takes from two to three hours, involves making an incision along the hairline that stretches from the temple, inside the ears, down around the lobes, and to the back of the neck.  Excess skin is removed, and the underlying muscles are re-draped to restore a more youthful face.  Dr. Kanodia believes that the most fundamental elements of a face lift are preserving the hairline as well as hair, and ensuring that scars are well-concealed.

Non-Surgical Facial Rejuvenation

There is beauty to be found in every human face. It is not the role of our science to gauge beauty against some arbitrary standard, but to utilize our tools to allow the various attributes of each face to work in concert. Consider the way a photographic image is made. Light falling on the subject is reflected from its contours and features through the lens of the camera. It is this reflected light that is photographed. I look at the face as if through a lens. The contours of a tired, wrinkly face do not reflect light in a harmon-ious, coherent way. I “adjust the light” until I am able to establish the youthful vectors in the face. The resulting “photograph” becomes my model. My work is to endow the face with qualities it never possessed and to bring out those qualities of beauty that are intrinsically its own.

Just as aging is an ongoing process occurring in distinct stages, our effort to reestablish the youthful appearance must also be gradual. The rejuvenation is accomplished in several sessions, according to the needs of the individual and the degree of change that is sought.

Facial Rejuvenation involves 4 necessary components. Just as a painter begins a portrait by selecting a palette appropriate to his subject, our work requires many different colors for its completion.

A) Fillers
B) Dysport and Botox
C) Lasers
D) Skin care

Fillers:
Anti-aging dermal fillers, including Juvederm, Restylane and Perlane are also staples that Kanodia’ says can halt the march of time. Once again, any positive or negative results, including the length of time a filler lasts, lies in the skill of the person giving the injections. Whereas some physicians give up the job of injecting fillers to their nurses, Kanodia does the injections himself to make certain the filler is placed in the exact anatiomical and asesthetic area to guarantee optimum and long-lasting results.
The safest filler is Hyaluronic Acid (H.A). It is injected not only to fill lines (e.g., Laugh lines and Marionette lines), but to volumize and to reestablish the youthful contour. Facial Rejuvenation Philosophy: When done with educated precision, Botox is a brilliant and powerful tool that eliminates wrinkles from the face. Lines between the eyes, in the forehead and those nasty crow’s feet’ by the eyes soften in about five days and the effects last several months. But when Botox is injected incorrectly, get ready for a big nightmare. Bad Botox injections cause uneven facial features and a startling plastic look that wipes natural expression from the face.

Dysport and Botox: (neuromodulators) relaxes muscles to soften and eliminate certain lines and creases. Contrary to some belief, Botox used properly does not “freeze the face” and eliminate its expressiveness. Here again, achievement of a natural-looking result is as much art as science. The objective is not simply youthfulness, but youthfulness in accord with the individual face. Botox injections should be done very gently and carefully so as to prevent damage and deep lines; it should be precisely calibrated to the needs of the patient. Primary Sites

1. Frown Lines (Procerus and Corrugator)
2. Horizontal Lines in the forehead (Frontalis)
3. Crows Feet (Orbicularis Occuli)
4. Neck (Platysma)- to prevent downward pull

Some degree of eyebrow lift can be accomplished by creative and selective use of botox between area 1 and 2 and 3.

Secondary areas of benefit from botox are:

Abstract (page 3)

1. Vertical lines above the upper lip (very carefully and superficially injected with micro drops using a 32 gauge needle). Here I use 1-2 units, causing no interference with the mobility of the mouth.

2. Mentalis–in the chin to soften the orange peel appearance.
